The Live Active referral scheme is a joint partnership between Inverclyde Leisure and NHS Greater Glasgow & Clyde.

Since its launch in 1997, our focus has been on helping people increase their level of physical activity, and gain core skills to lead an active life.

Live Active, Inverclyde Leisure’s GP Exercise Referral scheme is the perfect way for people with or without long-term health conditions to move more to improve their health and wellbeing.

We want everyone to be able to enjoy the benefits of a healthier more active lifestyle. The current Government physical activity guidelines are ‘to accumulate a total of 150 mins of moderate intensity activity per week’. This can be achieved in various ways and as often as you wish. We are here to help you.

The Live Active referral scheme is a joint partnership between Inverclyde Leisure and NHS Greater Glasgow & Clyde. Since its launch in Inverclyde in 2008 our focus has been on helping people who are inactive to increase their level of physical activity, and gain core skills to lead an active life.

The Live Active scheme aims to provide opportunities for people with underlying medical conditions, or those who are inactive and at risk of developing conditions to become more active in a safe and welcoming environment and raise awareness of the benefits of physical activity, helping to create long-term behaviour change.

Regular physical activity can help you manage your weight, have a positive effect on you mentally, improve your muscle and joint functions, and benefit your heart health too. Not only will it help you manage and control existing conditions, but it will help protect you from developing other health complications as well.

WHO CAN BE REFERRED ONTO THE GP EXERCISE REFERRAL SCHEME?

Live Active is also suitable for those who are inactive and do not have a long-term condition but feel they would benefit from advice and motivational behaviour change support on their physical activity journey.

Most people with long-term health conditions will be accepted onto the GP Exercise Referral Scheme. These include asthma, arthritis, high blood pressure, diabetes and obesity, as well as depression, anxiety, osteoporosis and plenty of other conditions, including rehabilitation from Covid-19.

The programme has been designed to help individuals self-manage their condition more effectively and with more confidence.

 

HOW CAN I JOIN THE SCHEME?

  • Simply speak to your GP, nurse, physio or other health care practitioner
  • If you are eligible, you will be referred to us

 

ELIGIBILITY CRITERIA

The scheme is aimed at inactive adults, 16 years and over with a low or moderate risk long-term condition.

If patients have been regularly active for more than three months, two or more times a week, they will not be accepted onto the scheme. They may, however, be recommended alternative membership options.

 


 

WHAT DOES THE GP REFERRAL SCHEME INVOLVE?

After being referred to the Live Active Exercise Referral Programme by your health care practitioner you will be contacted by one of our Live Active Advisors who will arrange to meet up with you for an informal consultation.

During this time they will chat with you about your own personal aims and goals, health requirements, exercise history and help you to overcome any barriers (if you have any!) before offering a wide variety of suitable activities to help you to take the first steps to a healthier more active lifestyle.

Whether you choose to attend a specialised Vitality exercise class, supervised fitness gym session, walking group or individual activity such as gym, swim, walk, home-based activity we are happy to support you on your journey! Follow up support will be provided throughout the 12 month programme within supported activity sessions or via one to one consultations over the telephone or in person.
