Live Active Referral Scheme

The aims of the Live Active Referral Scheme are:

To assist those who would benefit most to take the first step towards being more active, and make small positive changes to their diet, supporting them to live more healthily.

The process is as follows:

Potential participants are identified by their GP, Practice Nurse, Physiotherapist or Cardiac Physiotherapist, a health profile is completed detailing relevant medical history and sent to the Leisure Centre of their choice. The Health Counsellor at their local leisure centre arranges an initial appointment, if the referral is appropriate they will be enrolled onto the scheme for 12 months.

The benefits are that participants on the scheme receive:

A tailored plan to suit the individual needs of the client and ongoing support and advice. 

On completion of the scheme participants should feel more confident to continue with their activity plan and no longer require the support from the Health Counsellor.
